12 Grade Vaccines (Class of 2022)

Class of 2022 Students, Parents and Guardians:

Please review important information below about required vaccines for students entering 12th grade next school year. ~Mrs. Fromm

11th grade Parents and Guardians:

Please be advised that immunizations must be current by the 14th day of each new school year. If vaccines are not up-to-date, your child may be excluded from attending school. 

Please take the time this summer to make sure your student is ready to enter school in August.

Here are the requirements:

Students entering 12th grade: 

Must have 1-2 doses of Menactra (Meningococcal/MenACWY).

If a student receives their first dose of the vaccine on or after their 16th birthday, a second dose is not required.

If the first dose was received prior to the 16th birthday, two doses are required. 

Proof of these vaccinations must be submitted to your school nurse by the third week of the 2021-2022 school year.

If your child cannot get these vaccinations due to medical/religious/personal beliefs, you may request a waiver form.
